What frightens On your first day as a fixer-upper for the Ministry of Happiness and Peace, you operate on sedated citizens: scrubbing skin, setting limbs, pulling teeth and pinning their faces into a smile, while an intercom voice warns that if you make too many mistakes you will be the one fixed next.

Content: blood, gore, body horror, non-consensual surgery, jumpscares, mentions of death and violence.

A free, short medical horror game. As a government fixer-upper you operate on sedated patients, fix their defects and pin their faces into the one approved expression, while hints of rebellion tempt you away from the Ministry's orders toward different endings.

About Smile More

Smile More is a short point-and-click horror game by Pixel Poxet and a small team, first made for the 2026 Global Game Jam and released free on Steam in March 2026. It is also on itch.io as a pay-what-you-want download.

It is your first day as a fixer-upper for the Ministry of Happiness and Peace. Each patient arrives sedated with a file. You read it, inspect the body for defects and mark them down, then use your tools to close wounds, set broken limbs, remove unapproved marks and fix teeth. Last of all you take pins from a pincushion and fix their face into a smile, fill in the Patient Defect Status sheet and hand it in at the terminal.

The horror is both on the table and around it. The surgery is drawn in grainy purple and red with plenty of blood and body horror, and the store warns of jumpscares. The Ministry speaks only through your intercom, tells you to discard any sign of rebellion and to ignore whatever the patients ask of you, and makes clear that you can be fixed too. Whether you obey or keep looking leads to different endings.

Players praise its art, atmosphere and voice acting and wish it were longer, and some find the clicking finicky. It suits players who liked operating-table horror and dystopian job games and want something free for an evening.

How to play Smile More

  1. Download it free from Steam or itch.io on Windows or Mac.
  2. Read the patient's file to learn who they are.
  3. Inspect the body and note every defect.
  4. Use your tools to fix wounds, limbs, teeth and marks.
  5. Pin the face into the approved smile, fill in the defect sheet and turn it in at the terminal.
