Fear & Hunger is a horror RPG that refuses to be fair: one of four adventurers goes down into an ancient fortress where gods war over planes of existence, hunger and sanity drain away, and turn-based fights can cost you an arm. These games share some of that: turn-based fights in a horror world, survival that grinds you down, or a story that goes to very dark places.
Start with the series itself. Fear & Hunger 2: Termina throws fourteen strangers into a deadly festival in a backwater town, with three days to learn its secrets before a grinning green moon reaches its apex and only one survivor remains. It is better liked on Steam than the first game and just as extreme.
For the same mix of survival and turn-based fights, Look Outside is the big recent hit: a survival horror RPG set in one apartment building after everyone who looked out a window became a monster, where you scavenge, fight and decide which neighbours to trust. Trench Face sets turn-based survival in a First World War trench full of corpses, with hanging monsters patrolling the dark, and WORLD OF HORROR turns cosmic horror into a 1-bit roguelite of mysteries and fights, with stamina and reason that run down as doom creeps closer. Labyrinth Of The Demon King is the first-person cousin: a dungeon crawl through demon-haunted feudal Japan with scarce supplies and saves only at shrines. Darkest Dungeon is the best known of the gothic ones: you send teams of four flawed heroes into the dungeons under a ruined estate, where disease, hunger, the dark and their own stress drive them toward madness, and death is permanent; Darkest Dungeon II takes four heroes by stagecoach across a maddened land. Psychopomp is the short, free one, a step-by-step crawl through catacombs among grotesque creatures you can talk to, touch or smash.
For the strangeness, OFF is a surreal 2008 RPG, remade in 2025, in which a silent man with a bat purifies zones haunted by specters, and OMORI hides sudden horror and a story of grief and guilt inside a bright dream world. Blackout Hospital is a six-to-eight-hour RPG Maker game of a janitor trapped in a failing hospital, and Pathologic 2 makes survival itself the horror: twelve days as a plague town's healer, where hunger, exhaustion and infection count against you. The Coffin of Andy and Leyley is not an RPG, but it goes as far into taboo as Fear & Hunger does.
